Contacting our office
Our phone lines are open 8 a.m. – 5 p.m., Monday – Thursday, and 8 a.m – 4 p.m. Friday, except for holidays. For your convenience, many common requests can be addressed through our patient portal. It is quick and easy to create an account!
For medical emergencies or if you are having chest pain, call 9-1-1.
If you are experiencing an urgent problem, please contact us at (480) 945-3535. If it is outside of regular business hours, our answering service will contact the Doctor on call.
Making an Appointment
For your convenience, you may also request an appointment through our patient portal. If you are not a member of our patient portal, please call the office to request an Appointment Scheduler. Please be prepared to provide basic demographic and insurance information when scheduling an appointment.

Canceling an Appointment
If you are unable to keep an appointment, please call us at least 24 hours in advance so we may make the time available for other patients. You may cancel by phone or through the patient portal. Please note: Failure to notify the office may result in a non-cancellation fee as some procedures require the additional purchase of third-party supplies.
Arrive on time
Please arrive for your scheduled appointment 15 minutes early for check in. Bring with you photo ID, Insurance Card and your medications or current list of medications.
Patients who arrive more than 10 minutes after their scheduled time may be worked into the schedule and seen as soon as possible, or there may be a lengthy wait, or your appointment may be rescheduled depending upon the schedule for the day.
As a precaution, if you are running late, call (480) 945-3535 to notify us of your delay.
For your convenience, we e-prescribe. (Electronically send your pharmacy your prescription) For the most efficient service, please call your pharmacy to request the refill.
You may also submit prescription questions and requests through our patient portal. Please allow 48 hours for all prescription refills.
Request Test Results
If you have not heard from us within 3-5 business days, please call our office.
If your test was ordered by another physician outside our group, please call the ordering physician’s office for the results.
Questions About Your Bill
Your insurance company is the best resource for most billing questions. If you have not done so already, please call the number on your insurance card. If your insurance company cannot resolve the issue, please submit your question via our patient portal. Our billing specialist will get contact you as soon as possible. If you are not a member of our patient portal, please call our Billing Office via our main number 480-945-3535. Your call will be routed to a Billing Specialist’s extension. If the Specialist is currently with another patient, please leave a voice mail message and your call will be returned in the order in which it was received.
